Well, its a long story, full of twists and turns, action and adventures. There are even a few scary bits. But seeing as you asked why dont you pull up a metaphoric chair around my metaphoric fire and Ill tell you the story of Black Milk â€Å" When James was young, there was a time that he was broke, and everyone around him seemed to have a great job that they enjoyed, â€Å"I was broke, bored and rather cold. At that point most people would have got a job and a beanie, but not I†. He woke up one morning with a desire to make himself a shirt, he went out and bought a second hand sewing machine, and with only $6 left in his wallet, he bought the cheapest nylon lining that he could find. He still does not know why he had such random impulse but he is very glad he did, â€Å"It was these fateful decisions that lead me down the path to Black Milk†. Nylon lining is one of the least attractive fabrics in the market; he had to make a fitted shirt with no one to teach him how to sew this complicated fabric. â€Å"It was tragic. The stitching was wrong, the pieces were cut weird, and the neck was a nothing more than a hole. And I loved it† From that moment, things changed. He began doing odd jobs, but â€Å"at the back of my mind I was plotting and planning. If I got ten bucks it wasnt just ten bucks it was half a meter of printed fabric. Three dollars was a new packet of needles. A hundred bucks meant I could get my machine fixed. † With all his enthusiasm he worked in trial and error testing different ways of sewing bits and pieces of fabric. â€Å"You will immediately notice: 1. Both legs are too short 2. The pattern on the right leg is upside down (Hey, Im a slow starter, alright?! ) However, despite this every girl who tried them on wanted to buy them. Which was super encouraging for me. So I kept going. † After one week of teaching himself how to sew with a half working machine, he went into a dance shop and bought some shiny sparkly fabric. â€Å"The shop was full of mothers, grandmothers, and me. They looked at me like I was from another planet†. He bought a tribal African print because he had seen on the shops that these prints were in fashion, he thought about the fabric and felt an intuition that it would look best on legs. He started working immediately, drew up a  pattern, cut it out and made a pair of leggings in an impressive time for a starter. He wanted to try the leggings on a model and talked to his friend who had a friend who agreed to be a model for James to see if they fit. She put them on, and they didnt. So he cut them again, and sewed them again several times. And they still didnt fit. â€Å"I must have recut those things ten times†. And finally, they fit. It was at that point that something remarkable and completely unexpected happened, this changed everything. She asked if she could buy them. She took out her wallet, gave him some money and walked off with James leggings. â€Å"I couldnt stop smiling for the rest of the day. † It was at that point that James felt that it was not impossible. â€Å"I knew that if there was one girl prepared to buy one pair of my leggings, there were (probably) a hundred girls out there who would do the same†. He felt even more confident and he worked even harder to start his own little business â€Å"I threw myself into it with a passion. I would spend 5-6 hours a day on the sewing machine trying to figure out how to sew properly. I even attended sewing lessons at the local sewing shop (yes, I stuck out like a sore thumb). It was pretty tough in those early days. On more than one occasion I came close to throwing everything in the bin. † The next in the list was getting out and selling his designs, so he went door to door, trying to get shops to sell a few of his pieces. And after several painful no’s, one place finally agreed to display a few of James clothes in a corner of their shop. Unfortunately, a week later and they gave him every single piece back. But he was very determined and bought markets to sell his clothes in the Saturday markets. Since 5:30am every single Saturday, James stood around under the sun all day, selling his creations. He was considerably unpopular at the markets, but hundreds of girls all around the world were very interested when they found his clothes through blogging, that soon became much more famous than the tent and so James decided fatefully to make Black Milk exclusively into an online fashion label. â€Å"I talked to a few people in the industry who told me I was making a big mistake and that I would go broke within the month. I was told that online selling was only for companies with real shops. But real shops werent interested in my clothes, so I went online. As it turns out, it was a very good idea. :)† Within 3 years Black Milk have gained over 15000 fan on Facebook been so huge that the team can’t keep up with the sales and a lot of â€Å"sharkies† (Black Milk fans) are getting disappointed on the low reach of the company. James is working as fast as he can with the team to improve his production. Black milk is selling products all around the world, but James himself blogged that the top countries currently buying black milk are â€Å"So what the heck maybe well just go nuts and set up a studio in the US. You only live once. And of course, we will still have our Australian studios and they will keep expanding. 🙂 Of course, it goes without saying that our strict policy about not manufacturing in the third world is not going to change any time soon. In fact never. :)† â€Å"I dont know what this hand signal means, but if it is something along the lines of Guess what Black Milk did a deal with LucasArts and so the most awesome clothing ever created will be available Soon then it is surprisingly accurate† James is very excited that the industry grows bigger every day and very fast. â€Å"Instead of me sewing away by myself at night, we have a fantastic team who pump out thousands of garments every month. Instead of trying to get our clothes into real shops, were online and loving it. Instead of selling to a few girls at the markets, we now have a community of the most amazing girls you could hope for who are passionate about the brand. However, there are some things that havent changed. I still love making clothes. I still love hearing from girls who buy the clothes. I love seeing all the different ways girls style their Black Milk pieces, and I love working with all the Black Milk team. At this time it was very rare for a Spanish or Italian designer to produce working drawings from their designs. Concept sketches serve only to convince the designer himself and his assistants within the studio that a design proposal is possible and to develop the main idea; but Blanca prefers to strike up face to face discussions with those responsible for manufacture and designs may be modified many times before the shape and form of the product is finalized. Blanca believes that there are two types of technicians, â€Å"those who only see difficulties and the usually older craft-men who say ‘this may seem impossible but if I make it this way perharts it is possible’ if a designer does not propose silly things the technician will respect him, it is not possible to improvise design†. His success consists in insistent supervision, not only because he prefers to build a comfortable environment for everybody but because he believes that â€Å"if the design is a failure it is completely my fault†. He worked for several companies a lot of them owned by personal friends, he was highly attached to the technicians â€Å"they are clever and full of suggestions rather than coercive†. But he also had his up and downs, such as the disagreement with the furniture manufacturer â€Å"knoll international† which Blanca believed was overdoing the suggestion that he submit three separate design solutions for a chair. Blanca found the task â€Å"impossible to come up with† because he was a strongly innovative man and in Spain at the time, the market was cost effective and manufacturers believed cheaper to copy Italian and Swedish designs, finding a cheaper solution than that, required a bit more than cleverness including the fact that the manufacturing had to be as cheap as possible and he found the deal with the rest of the workers very hard to accept because it went against his principles. Finding one solution only was hard enough. However soon enough, Spain entered the European Community and the industry around Barcelona and Valencia begun to change as imports tariffs had dropped. It was no longer cost effective, the Italian and Swedish designs were put aside because Spanish consumers could now afford the products of competitive wares which started looking for designers. Blanca had a lot of enthusiasm in trying new inventions which he would not have had the courage to try with Italian producers. The range begun in 1984 and comprises seven different chairs. The lattest was designed for the refurbishment of Domenech I Montaners 1903 previously mentioned â€Å"Palau de le Musica Catalana† in Barcelona. It is described as an auditorium chair and much study has been applied to the acoustical problems â€Å"a good chair for a cinema is a bad chair for a conference hall† This was the final solution, designed for auditoriums. It was first used when renovating the Palau de la Musica Catalana, in Barcelona. However this design was so successful, that if a designer saw it, would think it had been evolving and being perfectioned for years rather than the truth, a lucky clever design. Underneath each seat users find a Helmholtz resonator, which, absorbs the same amount of sound as a human body when the seat is upright. The result is little difference of reverberation no matter how many people were in the hall. This design is said to be successful because is now being used in more than 50 public halls in Spain, including the Senate. This chair was only one of the innovative solutions that Blanca introduced to the â€Å"Palau de le Musica Catalana† that he spent six years enlarging, restoring and air conditioning. The In the first phase, which took several years to achieve, Blanca among with his co-workers, repaired the very damaged parts of the old building and equipped it with sanitary facilities, accessibility, safety and comfort features. These were of course unimaginable a hundred years earlier, but his team made it possible. Palau de le Musica Catalana. A Streetcar Named Desire by Tennessee Williams MITCH: Lies, lies, inside and out, all lies. Referring to the two critical opinions, explore the extent to which the relationship between Mitch and Blanche is based on deception and self deception. Throughout Williams' play an unexpected relationship is developing before us. This is the one formed between Mitch and Blanche. Two very different characters who would appear to have nothing in common but when they dig deeper into each others personalities they find that have shared many past experiences and this gives them a basis for their relationship. However, after realising that Blanche is not the same person that she appears to be, Mitch begins to doubt anything that she has told him in the past. Can the audience blame him, as they know that she has even lied to her own sister. What makes her relationship with Mitch different to that she has with any other person? What makes him so special that she does not feel the need to lie to him? Mitch proposes to Blanche that everything that she has told him is a lie. That the person that he knows her as is make believe. "Lies, lies, inside and out, all lies" (Scene 9), is the phrase used by Mitch to convey his feelings to Blanche. He is right. Blanche had based all her relationships on lies since she had arrived in Stella and Stanley's house. She felt that it was the only way that she would be accepted by her sister and brother in law. She was never expecting to meet such a man as Mitch, but once he did appear she had to continue the deceit with him, to make sure that she did not incriminate herself, as she had already spun so many stories for Stella and Stanley. Mitch is a very weak character. He stays at home... ...and she is not as hard faced as she has seemed throughout the play. All she wanted was to have something to replace what disappeared when she lost Allan. There had obviously been something missing since he died, and that is what Blanche had been searching for, sadly everywhere she looked she only found herself in trouble. Cardullo sees her actions at the Flamingo, not necessarily as prostituting herself, she may not have thought of herself as a prostitute as most people would define it. So you have to take care of your health, how you prepare yourself, and you always have to respect your opponents, and if you do that you will be a great player and no one will stop you. † This is one famous quote from Pele’s book â€Å"My life, and The Beautiful Game. † 2.Brief Intro- Edison Arantes do Nascimento, more known as Pele was one of, if not the greatest footballer in the world. He was born on October 23, 1940 in Tres Coracoes, Brazil. At only 15, he began to play professional football, and made a huge impact in his team, as well as the national team. Even though Edison was his real name, Pele caught on because it means a miracle in Hebrew. He did not like it at first, but got use to it. At first, he was glad that he could do anything he wanted and still look beautiful but near the end, the painting drove him to the edge. He tried to change his ways but it was too late. Characters (Gaby) So the main character in the novel is Dorian. Dorian Gray is a handsome and wealthy young man who serves as a muse to Basil Hallward. A dynamic character because he turns into a monster by the end of the book. He is the archetype of beauty, representing the main theme about soul corruption and beauty. Next we have the devil, Lord Henry Wotton. He is the cynical one and always arguing very controversial ideas which is probably why the novel was censored during that time. Many professors believe he was the representation of the devil because he incited Dorian to commit sinful acts and then would go and have a cup of tea in the afternoon. There is also, Basil Hallward with his puppy love. He is the artist and the one who paints Dorians portrait. Hes so in love with Dorian that is afraid of portraying his love in his painting. He tries and fails to save Dorian from the influence of Lord Henry and it will end up costing him greatly. Other important character is Sybil Vane. She is an actress from a poor theatre and Dorian falls in love with her young beauty and skills. This quote was used as an excuse for Dorian to not stop reading the book due to his curious-self. An excuse for Lord Henry sins. Behind every exquisite thing that existed, there was something tragic (narrator, chapter 3, pg. 41). The quote basically means that behind everything beautiful, there is something dark and tragic. Lord Henry says this after learning about Dorians past and his parents death. Despite his dark and tragic past, Dorian is still innocent and beautiful. The quote is also meant to foreshadow Dorians future and how his beautiful portrait is used to conceal his evil and atrocious actions. Laughter is not at all a bad beginning for a friendship, and it is by far the best ending for one (Lord Henry, chapter 1, pg. 6). This quotes foreshadowing the chain of misfortunate friendships in the story because laughing to end a friendship is not only mean but evil Okay, so lets start with our first quote. Dude,The books that the world calls immoral are books that show the world its own shame (Lord Henry, chapter 19, pg. 223). So he is obviously referring to the book, its called no spoiler. Laughter is not at all a bad beginning for a friendship, and it is by far the best ending for one (Lord Henry, chapter 1, pg. 6). Vocabulary time. Aphorism, an observation that contains a general truth. So, this aphorism means that humor is a good way to start a friendship. Critical reflection skills hold much value in the nursing field since they help in prioritizing major decisions and achieving better patient outcomes. Further, it can be referred to as an extension of critical thinking through which helps the nurse to solve queries by probing questions. In addition, it moves beyond examining the past experiences and focuses on applying the gained knowledge in the future (Mill et al. 2016). The present paper is a critical reflection written on the basis of a nursing patient care case study that aims to demonstrate critical ability. The paper focuses on four major aspects of nursing care for demonstrating critical reflection. First, it discusses how the integration of subjective and objective signs and symptoms and medications impact on the care provided to the patient. Secondly, it identifies the elements of care that are required due to the events in the simulation scenario. Further, it highlights the role of inter professional collaboration in this case study, lastly, it discusses the particular psychosocial issues arising in the case. The case study selected, from Case World, for the present critical reflection focuses on the patient named Adam Knight, a 20-year-old man who has suffered acquired brain injury after a car accident. The patient has deficits related to mobility, memory loss and the ability to engage in a social interaction with the peers. He is in need o f an effective community based long term support services as he is suffering from isolation. The focus of the case study analysis is acute pain and management strategies for the same would be highlighted in the paper (School of Nursing Midwifery 2014). Discussion How integration of objective and subjective signs and symptoms and medications impact care According to Morton et al. (2017), nursing care delivery is influenced to a significant extent by the integration of objective and subjective signs and symptoms and medications provided. From the case study and simulation, it has been found that Adam has undergone an acute injury in his head and fractures as a result of the accident and is awaiting CT scan. He is found to be in a drowsy state though the ICP is normal. As highlighted by Wintermark et al. (2015) a head injury is suffered by a patient due to trauma to the skull, brain or scalp. Such injuries might be either penetrating, in which the bone or skin can be broken, or closed, in which there is no cut to the skin. Head injuries lead to different symptoms whose cause is dependent upon the impact of the injury. Such symptoms might be developing immediately after the trauma or gradually afterwards. Head injuries are treated depending upon the type of injury suffered by the patient. The condition of the patient also influences the type of treatment that is to be decided on. It is crucial that an appropriate diagnosis tool is used for assessing the severity of he injury suffered to the head. From the widely used examination tools available for neurologic and physical testing, the commonly used test is CT scan. Computed Tomography (CT) scan of the head utilises special X-ray equipment for producing multiple images of the brain nd the head. This is done for detection of any brain swelling, bleeding, brain injury or skull fractures (Levin and Diaz-Arrastia 2015). The authors point out that CT scans are valuable under conditions when the patient is found to have loss of consciousness. In the present case, Adam has been found to be in a drowsy state. Following a head injury, there might be a damage suffered to the blood vessels besides the brain or to the brain itself. In case a blood vessel is damaged, there are high chances that blood is supplied into the brain. The most common feature of such a condition is drowsiness. The patient has undergone surgery to remove the left subdural haematoma and for repairing the right fractured tibia and fibula. The patient has been in much pain after he has regained consciousness and subsequently an increased dose of 4/24 analgesia has been ordered. Analgesia is used for relieving patients from pain. Pain management forms an integral element of patient care with the prime objective of the care plan to reduce pain level. The rationale for this is the fact that effective pain management ensures that associated patient complications are also handled in an effective manner (Song et al. 2015). The patient is also found to have right hemiparesis (School of Nursing Midwifery 2014). Hemiparesis is the condition wherein the patient has weakness on one particular side of the body due to medical complications such as trauma or injury as he has suffered vehicle injury. This condition needs to be looked into by the nurse for preventing further impairment to the body part (Mark and Taub 2017). Elements of care due to events of simulation scenario The nursing practice elements that would be considered for the reflection paper are acute adult injuries, acute pain and medication reaction. The patient has suffered acute injuries to his head. As highlighted by Morton et al. (2017) acute injuries to the head leads to frequent death or disability at large. Acute pain management is an essential component of nursing care services. Pain is a complex, subjective, and multidimensional concept which is referred to as a highly unpleasant experience (Lehne and Rosenthal 2014). Analgesia is commonly administered to the patients for relieving them from the pain, suffering and alleviating them form adverse psychological and physiological issues. In the present case, Adam has been having altered levels of consciousness, and thus acute pain management is imperative. From the case study, it is found that Glasgow Coma Scale (GSC) had dropped as per the handover and his RR is 8. The medication had been changed from subcutaneous morphine to fentanyl 500 mircograms/mizadolam 50 mgs in a 50 mls syringe infusion. Subcutaneous morphineinjections are more acceptable and comfortable as compared to intramuscular ones. Boluses of subcutaneous morphine are often more effective for acute severe pain than commencing an infusion. In addition, he had been administered Naloxone. In the present case, the GCS had dropped, indicating that the patient had been suffering from severe head injuries. As pointed out by Adams and Urban (2015) fentanyl is an excellent analgesic that acts as a local anaesthesia and has been proved to give better patient outcomes as compared to morphine. It is for patients who are tolerant to opioids. Naloxone is a drug used for reversing or blocking the actions of opioid medications that include loss of consciousness, slowed breathing and drowsiness. This medication was, therefore, suitable for Adam as he had been suffering from all these three symptoms. The low readings in RR are due to injuries in the lungs shortly after the injury to the brain and subsequent damage. The mechanisms include inflammation, pulmonary oedema and autonomic system dysfunction (Lehne and Rosenthal 2014). Role of inter professional collaboration The role of a multidimensional team is extremely important in such a scenario as Adam requires more than one type of treatment. Here, the diagnosis and treatment suggestion of the doctor as well as the therapeutic treatment of the nurses will not provide him with enough chance to get back into the normal life because of the different effects the brain injury had on the different aspects of his life. Besides, he also had surgery in his tibia and fibula which had also affected in his mobility. Moreover, the brain injury had resulted in his memory loss incidences and also in social exclusion. Recommendation Critically reflecting on the care process delivered to a patient is useful in understanding the means of delivering a comprehensive care. A comprehensive assessment would enure that all patient problems are highlighted adequately. Advanced medical diagnosis is the key to outline a suitable nursing care plan aligned with the needs of the patient. The prime nursing care elements arising form the case study are acute injuries, acute pain and oversedation. Acute injuries are associated with a trauma suffered by the individual wherein the body undergoes a negative change in the concerned area, leading ot multiple adverse reactions. In the present case, the patient had suffered acute injuries to his head. Acute pain is the prime nursing diagnosis as it is a protective function acting to inform the patient about the injury suffered. Medication reaction is a valuable care aspect since medications doses and administration route are to be modified as per the need of the patient. Safe and effec tive care of patients needs to include maintaining a balance between the provision of pain relief, monitoring for appropriate use of prescribed medications and other substances, and recommendations for viable treatment alternatives. Rehabilitation would be the key nursing approach for Adam. The nurse would need to assist Adam in adopting an altered lifestyle and provide him with appropriate resources to develop and implement a proper discharge plan. Coordination of nursing care activities in collaboration with other members of the interdisciplinary rehabilitation team to facilitate achievement of overall goals is important. A holistic approach is needed in this case. A multidisciplinary team aids in the complete and comprehensive care plan for the patient. After analysisng the treatment options and the results of the assessment done, it had been highlighted that the primary goal for the patient would be maximising his abilities to lead to a normal life. Each professional from the team would deliver care in a manner that addresses different issues the patient has been suffering from. The professionals involved in the care plan of Adam were doctor nurses, physiotherapist, occupational therapist, speech pathologist , neuropsychologist, and social worker. There are a number of psychosocial issues arising in the case study. Inability to communicate with peers lead to isolation and frustration that the patient might find difficult to handle. This can be aggravated by the fact that Adam is struggling to become independent. A sudden change of lifestyle affects the emotional well being of the patient as well. Nursing care for the patient must, therefore, include this aspect of care in order to provide a comprehensive care service. The goal of rehabilitation for the patient would be to assist him to overcome his disability in mobility for maintaining maximum functioning.
